    The Clinical Reasoning Layer drafts a structured SOAP-format note, surfacing the source segments alongside each generated section.

    The clinician reviews, edits, and signs. Edits are version-controlled.

    On signature, the note becomes canonical — the same record financial and operational layers act on.

    Audit trail records the model version, the source dictation segments, and the clinician's edits — institutionally reviewable.

Is the AI the author of the note?

No. The clinician is the author. The AI is the drafting assistant. The signed note is the clinician's clinical statement.

How is patient privacy protected during ambient capture?

Capture occurs only with consent recorded in the encounter. Audio is processed within the institution's data residency boundary and not retained beyond the documentation event unless the institution's policy specifies otherwise.
