AI Discharge Summary

AI discharge summary drafts the comprehensive end-of-admission document — diagnosis, course, procedures, medications, follow-up plan — from the longitudinal record of the admission, for clinician review and signature. In HealthOS, the discharge summary is generated against the same record the admitting team operated on, governed by the Advisory Principle.

What hospitals struggle with

Discharge summaries are time-consuming and frequently delayed; delays produce gaps in handover to community physicians, missed follow-ups, and readmission risk. The summary often gets written from memory, not from the record.

How HealthOS runs it

The institutional workflow.

  1. 01

On discharge readiness, the Clinical Reasoning Layer assembles the admission record — diagnoses, key events, procedures, medication changes, outstanding follow-ups.

  2. 02

A structured discharge summary draft is generated, with each clinical claim linked to its source event in the record.

  3. 03

The discharging clinician reviews, edits, and signs. Patient and primary-care recipient are notified.

  4. 04

The summary flows to the Patient Platform and to the community-physician inbox simultaneously.

  5. 05

Audit trail captures the model version and the clinician's edits.

Canonical layer

This workflow runs on the Clinical Reasoning Layer.

The Clinical Reasoning Layer is one of the layers of HealthOS. It is not a separate product; it is a property of the substrate. See the Clinical Reasoning Layer layer →

Frequently asked

Does this replace the discharging clinician's clinical judgment?

No. The clinician reviews, edits, and signs. The signed summary is the clinician's clinical statement; the AI provides the draft scaffold.

Can primary-care physicians outside the institution receive the summary?

Yes — through the patient's record, FHIR-compliant exchange, or institutionally-configured pathways.

Position

The discharge summary belongs to the discharging clinician. The system's job is to draft against the full record — fast, complete, and verifiable. Signed by the Veronara Intelligence Office.
